Cultural Case Manager
(Young people 18 – 21 years))
Position Description
(SOCIAL, COMMUNITY, HOME CARE AND DISABILITY SERVICES INDUSTRY AWARD 2010 Level 5.1)
Hours: 38 hours per week
Wage: SCHADS Award Level 5
Location: Caboolture based with outreach support across areas of the Moreton Bay region
Conditions: Subject to a 3-month trial
Vision Statement:
"To provide quality services and support to Aboriginal and Torres Strait Islander children, young people, adults and community in a safe and culturally appropriate environment towards individual, family and community goals"
Position objectives:
1) To provide holistic therapeutic case management to young people exiting care both independently and those remaining with carers in the Moreton Bay Regional Council catchment area.
2) The role is to provide culturally safe, trauma informed and strengths-based practice case management. The role uses therapeutic practices with non-clinical delivery in a mentoring / role modelling walk alongside model of care in response to the voice of the young people.
3) To assess and refer where specialist supports, and services are required to meet the needs of young people.
4) To work in a collaborative partnership with key stakeholders both internal and external. Qualifications and Training:
· Accredited qualifications in the child, youth and or community work areas with demonstrated case management ability and a minimum of 2 years’ experience.
· A current unencumbered driver’s licence.
· A current Working with Children Suitability Check Blue Card. No Card, No Start as per legislation.
· Current Senior First Aid or willingness to acquire.
· Knowledge:
· Sound knowledge of the issues and needs of young people in a contemporary setting as well as the historical, cultural, and sub-cultural associations and impacts.
· An understanding of the issues and needs of children and young people in care and family needs and impacts.
· Ability to learn and abide by the Jabani Jinna Indigenous Corporation Policy and Procedure Manual and funding / legislative requirements.
· Ability to work under the direction and supervision of the CEO.
Skills:
· Ability to engage, support and work effectively alongside Aboriginal and Torres Strait Islander, young people and families in an outreach setting.
· Ability to deliver mentoring within the requirements and within designated timeframes.
· Ability to recognize and report issues or concerns for young people to the CEO and Team Leader. JJIC policy and procedures see mandatory reporting of any form of child abuse.
· The ability to work as part of a team.
· Ability to work in collaboration with agencies and families / carers towards mutual goals and needs of children and young people.
· Effective oral and written communication skills including advocacy, mediation and negotiation.
· Ability to record case management notes and other reporting responsibilities.
· The recognition of and participation in continuous improvement and professional development as important factors in the workplace.
Experience:
· Experience in providing support for young people and carers.
· Demonstrated experience and ability to carry out duties including reliability and consistency in employment. Key Selection Criteria:
1) Tertiary or other recognised accredited qualifications or demonstrated experience in child, young people and caregiver case management.
2) Demonstrated ability to effectively engage, support and work alongside children, young people and families / carers within a support, advocacy and referral role.
3) Demonstrated knowledge of the issues, needs and barriers that affect children and young people today and the needs of families in a contemporary setting as well as the historical, cultural and sub-cultural associations and impacts.
4) Demonstrated ability to work as part of a team and to contribute to action learning, continuous improvement and organisational growth and harmony.
5) Demonstrated ability to use Microsoft Office and online platforms for reporting.
6) Current unencumbered drivers’ licence
7) Current Working with Children Blue Card QLD
8) Current Senior First Aid Certificate or the willingness to attain one in a timely manner.
