About the Role
As a Community Nurse within our innovative Virtual Care Team, you will provide high-quality, patientcentred care that supports individuals transitioning from hospital to home and helps prevent avoidable hospital admissions. This role blends clinical expertise with the flexibility of virtual technologies, enabling you to deliver responsive, evidence-based care directly into the community.
PLEASE NOTE: While this role utilizes virtual care technology, it is not a remote or work-from-home position. The successful applicant will be site-based, reporting to a designated location within the Central Coast local health district.
You will work collaboratively with patients, families, and a multidisciplinary team to assess needs, develop care plans, and implement strategies that promote recovery, independence, and overall wellbeing. With a focus on health promotion, early intervention, and continuity of care, your contribution will make a meaningful difference to the lives of those you support.

About You
- You hold a current Authority to Practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA), ensuring you meet the required professional standards.
- You have a passion for providing patient-centred care, with a strong commitment to improving health outcomes through innovative virtual care solutions.
- You are a skilled communicator, able to build strong relationships with patients, families, and healthcare providers to ensure safe, timely, and effective discharge planning.
- You hold a current Australian Drivers Licence with the ability to travel across the Central Coast
- You are highly organised and adaptable, with the ability to effectively use virtual technologies and evidence-based practices to support patients and manage complex care in a virtual environment
