What you'll be doing
The CNC Integrated Care – Planned Care for Better Health will provide clinical, education and consultative service to all areas of Integrated Care for both inpatient and outpatient areas. The role provides support to the district and the local community for holistic patient centred care for chronic and complex conditions management. This position also offers clinical and strategic support to Value Based Health Care streams as appropriate.

How to apply
To be considered for this position, please ensure you address the selection criteria in the application as thoroughly as possible:
- Current nursing registration with the Australian Health Practitioner Regulation Agency (AHPRA) with at least 5 years full time experience as a registered nurse.
- Post Graduate qualifications relevant to chronic and complex conditions inclusive of training and assessment to deliver education programs (or working towards).
- Demonstrated clinical expertise, excellent written and verbal communication and clinical leadership abilities in Chronic Disease and Complex Care management. Virtual Care experience also beneficial.
- Evidence of participation in own professional development and commitment to maintaining competence and enhancement of skill, willingness to apply and share professional expertise with colleagues.
- Strong demonstrated understanding of primary healthcare and stakeholder relationship management to build stakeholder engagement.
- Demonstrated knowledge and skills in quality improvement, technology, developing and presenting education programs, clinical procedures and policies to staff and the community, project management, implementation, monitoring and evaluation with effective interpersonal and negotiation skills.
- Demonstrated ability to adhere to the defined standards of ethical and professional conduct consistent with the current NSW Health Code of Conduct and/or relevant professional standards where applicable.
- Current unrestricted drivers licence.
