About the Role
As a Lifestyle Facilitator (Support Worker), you'll provide personalised support that promotes independence, confidence, community participation, and quality of life for people living with disability.
You'll work alongside Participants to develop life skills, build confidence, make informed choices, and work towards the goals identified in their NDIS plans.
Key Responsibilities
Your responsibilities may include:
- Supporting Participants with daily living activities and personal care
- Promoting independence, choice, dignity, and decision-making
- Supporting social, recreational, educational, and employment-related activities
- Facilitating meaningful participation within the community
- Implementing Positive Behaviour Support strategies where required
- Supporting Participants with complex needs in a respectful, person-centred manner
- Assisting with household tasks, including meal preparation, cleaning, and laundry
- Performing safe manual handling, including hoist transfers and wheelchair assistance
- Working collaboratively with families, allied health professionals, and other support networks
- Maintaining accurate and timely documentation
- Following abbaCare policies, NDIS Practice Standards, and relevant legislative requirements

About You

Essential Requirements
You will have:
- Availability to work weekday morning, afternoon, weekends and/or evenings
- A demonstrated commitment to person-centred support
- Respect for diversity, inclusion, and cultural safety
- A compassionate, reliable, and professional approach
- The ability to advocate for Participants while promoting independence and choice
- Excellent communication and interpersonal skills
- Experience supporting people with disability and/or mental health conditions
- Experience supporting Participants with complex behaviours and support needs
- Competency in safe manual handling techniques
- Experience working active overnight shifts
- The ability to work independently as well as collaboratively within a multidisciplinary team
Mandatory Requirements
Applicants must hold, or be willing to obtain:
- Queensland Green P-Plate or Open Driver Licence
- Reliable vehicle with comprehensive insurance
- Current Blue Card (Working with Children Check)
- NDIS Worker Screening Clearance (Yellow Card)
- NDIS Worker Orientation Module completion
- Current First Aid and CPR Certificate
- Evidence of COVID-19 vaccination — minimum two doses, where required
Desirable Skills and Experience
The following experience and knowledge will be highly regarded:
- Experience working within Supported Independent Living (SIL)
- Knowledge and experience implementing Positive Behaviour Support Plans
- Understanding of privacy, confidentiality, safeguarding, and duty-of-care principles
- Experience building positive and collaborative relationships with families and external service providers
