Lead and support a team of Care Partners to deliver coordinated, appropriate and safe client care
The Care Partner Lead provides expert input into assessment, planning and monitoring of services delivered under the Support at Home program, CHSP, Claims Conference and privately funded services. This role leads a group of experienced Care Partners and ensures that clients receive coordinated, appropriate and safe support aligned with best practice and regulatory requirements.
About You
As a key leader within the Care Partnerships & Programs team, the Lead oversees operational and care standards in line with aged care act, supports team capability and ensures integrated care planning for complex clients.
You are a collaborative leader who builds strong relationships and confidently works with a diverse range of stakeholders to achieve positive outcomes for clients, understands how to ensure Quality care services as well as sustainable services.
The role also maintains a relative caseload, providing direct support and operational oversight for clients with specialist needs.
Skills and Experience
Advanced care planning and risk and complaints management
Team leadership and staff supervision
Strong communication and documentation skills
Ability to coordinate care across multidisciplinary teams
Knowledge of aged care clinical governance and Support at Home program requirements
Experience working with older adults with chronic and complex needs
Familiarity with the Aged Care Quality Standards and care coordination frameworks
Uses sound judgement in managing clinical risk and client complexity
What We’re Looking For
Minimum of 3-5 years’ experience in at least one of the following Home Care Packages (HCP), Community Aged Care, Case Management or related industry
Experience leading a team
Experience managing complex client loads and/or coaching others to manage these
Qualifications in related fields including Social Work, Allied Health Assistance, Enrolled nursing, Community or Human Services at a minimum Diploma level or equivalent experience in aged care leadership
A good understanding of aged care funding programs and care planning
Risk and complaints management
