At Dovida, we provide personalised home care that helps people live safely and independently in their own homes. We’re looking for a Care Manager who is compassionate, organised and confident building genuine relationships with clients, families and care teams.
In this role, you’ll work closely with clients and their families to understand their needs, create tailored care plans, coordinate services and make sure care continues to meet their goals as circumstances change.
Key Responsibilities
- Build strong relationships with clients and families throughout their care journey.
- Complete assessments and develop personalised care plans that reflect each client’s goals.
- Coordinate services and work with caregivers and health professionals to deliver consistent, quality care.
- Monitor client wellbeing, review care needs and adjust support where required.
- Maintain accurate records and ensure services meet aged care quality and compliance requirements.
- Demonstrate resilience and embraces change with a positive, solutions-focused mindset.
About You
- Experience in aged care, Home Care Packages or Support at Home.
- Strong relationship-building skills and the ability to work well with clients, families and stakeholders.
- Exceptional administrative skills, with the ability to efficiently manage multiple priorities using Microsoft Office, SharePoint, CRM systems, and other business applications.
- A practical, team-focused approach, with the confidence to work independently when needed.
