Allegiance Heart & Home Care is seeking a reliable, compassionate and experienced Support Worker to provide ongoing in-home support to an older person living with dementia.
We are specifically looking for a support worker who has demonstrated experience caring for people living with dementia and understands the importance of patience, routine, reassurance, dignity and person-centred care.
The successful support worker will provide a combination of personal care, dementia support, daily living assistance, domestic assisstance and companionship.
Responsibilities may include:

Assisting and supervising with personal care
- Showering and personal hygiene support
- Dressing and grooming assistance
- Providing regular prompting and reassurance
- Supporting a consistent daily routine
- Medication prompting and assistance in accordance with the care plan
- Preparing simple meals and breakfast
- Prompting and encouraging adequate food and fluid intake
- Light domestic assistance and general household tasks
- Laundry and linen assistance
- Supporting safe mobility around the home
- Falls prevention and supervision
- Companionship and meaningful conversation
- Social and community access support
- Accompanying the client to appointments or activities when required
- Monitoring the client's general wellbeing
- Recognising and reporting changes in behaviour, cognition, mobility, appetite or health
- Completing clear and accurate progress notes following each service
This position is best suited to someone who understands that dementia support involves more than completing physical care tasks.
The successful applicant should be comfortable with:

- Maintaining familiar routines
- Encouraging independence without compromising safety
- Communicating slowly, respectfully and clearly

- Supporting a person who may forget whether daily activities, meals or medications have already been completed.
Applicants should have:

Previous experience supporting clients living with dementia
- Certificate III in Individual Support (Ageing), Aged Care, Disability or equivalent qualification
- Experience providing personal care to older people
- Current National Police Check
- Current First Aid and CPR certification
- NDIS Worker Screening Check or willingness to obtain
- Valid Australian driver's licence
- Reliable vehicle and appropriate insurance
- Current Australian working rights
- Strong communication and interpersonal skills
- Good written documentation and progress-note skills
- Ability to recognise and report changes in a client's condition
- Understanding of dignity, privacy, choice and person-centred care
- Patient and calm
- Compassionate and respectful
- Experienced with dementia care
- Reliable and punctual
- Observant and safety-conscious
- Comfortable working independently in a client's home
- Able to follow an individualised care plan
- Professional when communicating with family and the care management team
- Interested in developing a consistent, long-term care relationship with the client
