About the Role: We are seeking compassionate, reliable, and motivated Disabilities Services Officer to join our growing team to provide quality support to people living with disabilities and older Australians, helping them maintain independence and improve their quality of life.
Key Responsibilities:
- Assessing clients’ needs and developing and implementing support programs and plans to best support those needs;
- Developing individualized service plans in collaboration with clients, their families, and other stakeholders;
- Coordinating support services, ensuring clients’ receive the required care that best meets their assessed needs;
- Providing support services to clients through assessment, case-management, care planning, budget management, and service coordination;
- Monitoring care package client costs, ensuring services are provided within each clients’ budget, package and plan levels;
- Monitoring and reporting on the progress of clients and referring clients to agencies that can provide additional help;
- Assisting clients with challenging behaviours through participation in the development and implementation of individually designed behavioural management plans;
- Coordinating social support activities for clients to participate in community events and activities;
- Building and maintaining positive relationships clients and their families and participating in client case meetings;
- Maintaining accurate and timely records and ensuring all relevant documentation is completed.
Requirements:
- Diploma (Community Services/Disability or equivalent) or higher degree qualifications in related fields
- Current Working With Children Check (where applicable).
- Current First Aid and CPR Certificate.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
