About the Opportunity
Join a dynamic and supportive team delivering hospital-level and specialist care directly into patients' homes through Hospital in the Home (HITH) and Residential Aged Care In-reach (RAC-I) services.
Based at Newcastle Community Health Centre, you'll enjoy a role that offers the opportunity to make a genuine difference by helping patients remain well at home and avoid unnecessary hospital admissions.
This role will see you:
- Deliver acute and complex nursing care across home and aged care environments.
- Undertake assessments, care planning, medication management and clinical interventions.
- Monitor patient progress, identify deterioration and coordinate appropriate escalation of care.
- Work closely with medical officers & multidisciplinary teams making sound clinical decisions in the community setting.
- Educate and support patients, families and carers to promote independence and wellbeing.
