About the Role
As a Care Partner, you'll be part of one of our collaborative regional teams, supporting older Australians to remain living safely, independently and with dignity in their own homes.
You'll become a trusted point of contact for your clients, working closely with families, Care Partners, Clinical staff, Allied Health professionals and Rostering teams to coordinate services, solve problems and deliver exceptional person-centred care.

What You'll Be Doing

Building trusted relationships with clients and their families.

Coordinating services that support clients to live safely and independently at home.

Conducting care reviews and developing person-centred care plans.

Managing client budgets in line with the Support at Home program to maximise outcomes and funding.

Working collaboratively with Care Partners, Clinical staff, Allied Health professionals and Rostering teams.

Identifying risks and opportunities to improve client outcomes.

Advocating for clients and supporting them throughout their aged care journey.

Managing a varied caseload while delivering exceptional customer service.
