Position Summary:
We are seeking a passionate, clinically skilled, and quality-focused Registered Nurse (RN) to join our HAS team. This role supports older people to live safely, independently, and with dignity at home through high-quality, person-centred care. The RN will lead clinical assessments, care planning, risk management, clinical governance, and quality assurance activities, while working closely with participants, carers, and multidisciplinary teams. The role also contributes to audits, incident management, staff education, policy reviews, and compliance monitoring to promote clinical excellence, safety, and continuous improvement.
Main Duties and Responsibilities:
- Conduct comprehensive clinical assessments and develop person-centred care plans;
- Provide clinical leadership and guidance to Care Partners and Support Workers;
- Monitor, review and manage participant risks, including complex care needs;
- Participate in case conferences and contribute clinical recommendations to care planning;
- Act as the Clinical Care Partner for the Restorative Care Pathway;
- Provide nursing support for End-of-Life and Assistive Technology & Home Modifications (AT-HM) pathways;
- Educate participants and carers on health management and self-care;
- Maintain accurate clinical documentation and support clinical governance activities;
- Conduct clinical audits, provide feedback to staff and monitor compliance with quality standards; and
- Develop, implement and evaluate audit action plans and continuous improvement initiatives.
Selection Criteria:
Essential
- Current registration with the Nursing and Midwifery Board of Australia as a Registered Nurse;
- Clinical problem-solving skills and a commitment to delivering quality person-centred care;
- Knowledge and/or experience in the aged care industry for at least 3 years;
- Demonstrable ability to identify gaps in quality and compliance;
- Good spoken and written English, fluency in spoken Cantonese and Mandarin;
- Good communication, interpersonal, time management and computer skills;
- Strong understanding of the needs of people from culturally and linguistically diverse backgrounds;
- Possess valid National Police Certificate or is able to obtain the certificate prior to employment;
- Possess valid driver’s license and own vehicle; and
- Current First Aid certificate.
Desirable
- Experience in Government funded home aged care programs; and
- Experience in conducting quality audits.
Salary and employment conditions for the above position is as per Enterprise Agreement. Salary packaging is available. Discount child care fees are available for CASS’s long day care, before and after school, and vacation care services.
