As a Registered Nurse in Community Health, you will provide high-quality, person-centred care to patients in their homes and clinic settings, supporting them to achieve the best possible health outcomes while maintaining their independence and wellbeing.
This rewarding role offers the opportunity to work across a diverse range of services including wound care, acute and post-surgical care, Hospital in the Home (HITH), palliative care, catheter and drain management, trial of void (TOV), medication management, and advanced wound therapy (VAC) dressings. Working collaboratively with multidisciplinary teams, patients, families and carers, you will help deliver compassionate care that reflects our vision of Leading Care, Healthier Communities and our Nursing and Midwifery commitment to Compassionate Nursing and Midwifery Care Always.
Are You the Right Fit?
We are seeking an experienced and compassionate Registered Nurse who is passionate about providing person-centred care in community and outpatient settings. You will be a confident clinician with strong assessment, communication and care planning skills, capable of working both independently and as part of a multidisciplinary team.
To be successful in this role, you will bring:

Experience in acute care, community nursing, or a combination of both settings.

Proven ability to assess, plan, implement and evaluate evidence-based nursing care.

Experience or exposure to services such as wound management, post-acute care, HITH, palliative care, catheter care, medication management, or advanced wound therapies.

Excellent communication and interpersonal skills, with a commitment to person-centred care.

A commitment to ongoing professional development and the NSW Health CORE Values of Collaboration, Openness, Respect and Empowerment.
