Disability Support Worker – Weemelah NSW

Assigned Guardian Angel is seeking a compassionate and reliable Disability Support Worker to provide one-on-one support to a participant in Weemelah, NSW.

Current Support Schedule
Monday – Home Exercise Program
- Approximately 1 hour of direct support
- Minimum 3 hours paid
Wednesday – Physiotherapy Appointment
- Approximately 4.5 hours
- Support to attend physiotherapy
- 4.5 hours paid
Friday – Home Exercise Program
- Approximately 1 hour of direct support
- Minimum 3 hours paid
Total: Approximately 10.5 paid hours per week
Although Monday and Friday require approximately 1 hour of direct participant support, the successful Support Worker will be paid a minimum of 3 hours for each of these shifts.
About the Role
Your responsibilities may include:
- Supporting the participant with their home exercise program
- Supporting attendance at physiotherapy appointments
- Personal care and daily living assistance where required
- Manual handling and mobility assistance
- Community access and social participation
- Encouraging independence and confidence
- Providing companionship and positive engagement
- Completing progress notes and relevant documentation

- Available Monday, Wednesday and Friday
- Comfortable providing personal care where required
- Confident supporting mobility needs
- Able to build positive and respectful rapport
- Professional and a clear communicator
- Committed to providing person-centred support
Requirements
- NDIS Worker Screening Check
- Current First Aid & CPR
- Current driver's licence
- Reliable vehicle
- Previous disability/support work experience preferred
- Manual handling experience or training is highly regarded
