Need2Heal is a nurse-led organisation providing Hospital in the Home (HITH), post-acute care, NDIS, Support at Home and private nursing services throughout Melbourne.
We currently support patients through multiple public Hospital in the Home (HITH) services, delivering high-quality clinical care in the community and helping patients safely transition from hospital to home.

About the Role
Working across Melbourne's northern, western and inner eastern suburbs, you will provide high-quality nursing care to clients in their homes.
This is an autonomous and rewarding role where no two days are the same. You will work with a diverse client group including HITH, post-discharge, aged care and NDIS participants, delivering a broad range of clinical services.
Clinical Services Include
- Complex wound care
- VAC (Negative Pressure Wound Therapy) dressing changes
- Prevena dressing management
- Catheter management (IDC/SPC)
- Continence assessments and management
- Stoma care
- IV antibiotic administration
- PICC management
- Portacath maintenance and chemotherapy disconnects
- Chronic disease management
- Palliative care nursing
- Clinical assessments and monitoring
- Post-acute and hospital substitution services
Key Responsibilities
- Deliver high-quality clinical care in clients' homes
- Complete comprehensive nursing assessments
- Develop and implement nursing care plans
- Liaise with hospitals, GPs, allied health professionals and support services
- Monitor client wellbeing and escalate concerns where required
- Provide education and support to clients, families and junior staff
- Maintain accurate clinical documentation
- Contribute to positive client outcomes and continuity of care
About You
To be successful in this role, you will have:
- Current AHPRA registration as a Registered Nurse (Division 1)
- Minimum 3 years' experience in HITH, Emergency, Acute Care, Critical Care, Community Nursing or similar
- Strong wound management skills
- Experience with IDC/SPC management (preferred)
- Excellent clinical assessment and decision-making abilities
- Strong communication and documentation skills
- Ability to work independently and manage your own caseload
- Current Victorian Driver Licence
