This is a fully compliant service with strong systems in place, allowing you to focus on delivering excellent resident care while being supported by experienced clinical leaders. About the opportunity Act as a primary contact for clients of CHSP and other programs, supporting clients clinical assessments Conduct comprehensive, person-centred assessments of client needs, suitability, and eligibility for Aged Care Services. Triage clients based on vulnerability, urgency, and level of need to ensure appropriate prioritisation. Identify and manage risks to clients and staff during intake and review processes. Complete scheduled and reviews for existing clients. Maintain accurate, timely records and reporting in line with the CHSP Funding and Program.
