The Team & Role
The EIiCD Diabetes Nurse Educator provides high-quality diabetes education, care, and clinical advice to clients within a community health setting. In this role, you will deliver evidence-based diabetes education and support across the continuum of care, including for newly diagnosed clients and those with more complex needs. You will support self-management and behaviour change through education, motivational interviewing, and health coaching, while also providing clinical support such as insulin stabilisation and the use of diabetes technologies, including continuous glucose monitoring.
The role involves undertaking comprehensive client assessments and developing person-centred care plans, including making referrals where appropriate. You will work collaboratively with multidisciplinary teams to ensure coordinated and holistic care, and support clients in lifestyle modification, medication management, and ongoing monitoring of their diabetes. Education will be tailored to meet individual needs, taking into account cultural backgrounds and varying levels of health literacy.

Skills & Experience Required
We anticipate you will have:
- Registered Nurse with current AHPRA registration
• Appropriate post-graduate qualification (certificate in diabetes education).• Experience in the provision of diabetes education.
• A current driver’s licence.
