About the role:
Practice with a high level of autonomy within your endorsed Nurse Practitioner scope, utilising an agreed medication formulary to deliver timely, patient-centred care that supports early intervention, reduces unplanned admissions and safe community discharge.
Deliver advanced clinical leadership and specialist nursing care through both direct and indirect services to patients, providing expert nursing assessments while supporting and educating patients to effectively manage their chronic health conditions.
Work collaboratively with the multidisciplinary team to drive evidence-based decision making, enhance patient outcomes, and lead service development initiatives that promote clinical governance and high-quality care delivery.
Participate in research opportunities to advance nursing practice and patient care, while fostering a supportive workplace culture through mentoring and promoting education, research and workforce development.
About you:
You're a highly qualified and endorsed Nurse Practitioner, holding a master's in nursing, meeting the Nursing and Midwifery Board of Australia standards, including the equivalent of three years (5000 hours) of full-time advanced clinical practice in Chronic Disease within the last six years, with specialist experience in Respiratory and/or Chronic Wound Care.
You hold registration as a Nurse Practitioner with the Australian Health Practitioner Regulation Agency (AHPRA) with a current annual practicing certificate.
You deliver and coordinate person-centred care, using advanced clinical skills to assess, plan, and manage complex conditions while ensuring safe, effective, and high-quality outcomes within your approved scope of practice.
You bring advanced clinical and technical expertise in chronic disease management, paired with strong problem-solving and interpersonal skills, enabling you to analyse service data, apply continuous improvement principles, and design evidence-driven models of care that optimise patient outcomes.
What we offer:
Total remuneration value of up to $187,710 p.a. is comprised of:
An hourly rate between $79.47 - $82.97 p.h.
12.75% employer superannuation contribution
