1. Our Support Coordinator connect people with disabilities with essential services, housing, and community supports.
2. Our support coordinator will empower clients to maximize their funding, navigate complex systems like the NDIS, and build independent living skills.
3. They will:
- do budget tracking, provider negotiation, and crisis management.
- play an integral role for NDIS participants in interpreting their NDIS plan, assisting them to establish the supports in their plan and develop their capacity over time to coordinate and manage their own supports.
Other Responsibilities
4. Service Connection: Researching and linking clients with suitable providers, community groups, and mainstream resources.
5. Plan Implementation: Assisting clients in understanding their specific support packages (e.g., NDIS plans) and developing strategies to achieve personal goals.
6. Capacity Building: Coaching clients to build skills, track budgets, and independently manage service agreements over time.
7. Crisis Management: Resolving points of crisis or service disruptions and fostering resilience in the client’s support network.
8. Compliance & Advocacy: Monitoring the quality and safety of services, reporting risks, and advocating for the client's rights.
Pay: From $52,000.00 per year
