Role description
Heathcote Health is seeking a compassionate and experienced Social Worker to deliver high-quality services across acute, aged care and community settings. Working as part of a multidisciplinary team, you will support clients, families and carers through assessment, counselling, advocacy and care planning, helping improve health and wellbeing outcomes across our community.
Key Responsibilities
- Deliver evidence-informed Social Work services across hospital, aged care and community-based programs.
- Apply a Social Model of Health approach to assess client needs and develop person-centred care plans.
- Provide short-term counselling for clients experiencing chronic disease, rehabilitation, mental health and other challenges.
- Advocate for clients, carers and families to ensure access to appropriate supports and services.
- Participate in multidisciplinary case conferences and collaborative care planning.
- Maintain accurate clinical records and contribute to service development and continuous quality improvement.
- Build and maintain productive relationships with health professionals, community providers and support agencies.
About you
To be successful in this role, you will have:
- A recognised university qualification in Social Work.
- Eligibility for membership with the Australian Association of Social Workers (AASW).
- Current Driver Licence.
- Current NDIS Worker Screening Check and Working with Children Check, or ability to obtain.
- Excellent communication and interpersonal skills with the ability to develop strong working relationships.
- Experience working across a range of Social Work practice areas, particularly with older people.
- Experience working within multidisciplinary teams to deliver person-centred services.
- Strong counselling skills and a commitment to evidence-informed practice and professional development.
