Multi Stream Patient Navigator
Ongoing, Full-Time Role – 80 Hours per Fortnight (with a Monthly ADO)
About the Role
Our Patient Navigators are key members of the administrative team, working closely with clinicians to support patients throughout their care pathway.In this role, you will:
- Be the first point of contact for patients, carers, and community referrers — responding to questions, resolving appointment issues, and helping to clarify instructions.
- Coordinate communication between patients and their clinical care team, ensuring timely and clear information flow.
- Facilitate patient access by managing patient flow and assisting with navigation through the cancer care system.
- Work within a tumour stream model, across multiple tumour streams, allowing you to develop specialist knowledge and build strong, ongoing relationships with patients and their multidisciplinary care teams.

About You
You will bring:
- Strong interpersonal and communication skills, with the ability to engage compassionately with patients and families.
- Excellent organisational and administrative abilities, including managing competing priorities.
- Experience working in a healthcare, administrative, or customer service role (healthcare experience highly regarded).
- A proactive and collaborative approach to problem-solving.
Remuneration under the Health and Allied Services, Managers and Administrative Workers (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2021-2025 will be from HS Grade 2 Year 1 (HS2) $66,768.00 per annum (full-time equivalent - to be prorated for part-time) to HS Grade 2 Year 5 (HS21) $73,811.92 per annum (full-time equivalent - to be prorated for part-time) dependant on candidate experience.
