Key Responsibilities
- Provide clinical leadership, advice and oversight across community aged care services.
- Act as Key Personnel and actively participate in AAC’s clinical governance framework.
- Undertake and oversee clinical assessments, care planning and reviews for clients with complex or changing needs.
- Identify, assess and manage clinical risks and ensure appropriate escalation and intervention.
- Provide clinical guidance and support to Care Partners, nurses and Community Care Workers.
- Review clinical incidents, adverse events, risks and trends and contribute to continuous improvement.
- Participate in quality audits, clinical governance, quality and risk meetings.
- Contribute to clinical policies, procedures, audits and quality improvement activities.
- Liaise with GPs, hospitals, allied health professionals and other healthcare providers to support coordinated care and safe transitions.
- Support staff education, clinical competency and high-quality clinical documentation.
- Support compliance with the Aged Care Act 2024, strengthened Aged Care Quality Standards, Support at Home requirements and other relevant regulatory obligations.
About You
You will have:
- Current AHPRA registration as a Registered Nurse.
- Significant clinical experience, preferably in community aged care, community nursing or aged care.
- Strong clinical assessment, complex care planning and critical-thinking skills.
- Experience or sound knowledge of clinical governance, quality and clinical risk management.
- The confidence to provide clinical advice and leadership across multidisciplinary teams.
- Knowledge of contemporary aged care legislation, quality standards and regulatory requirements.
- Excellent communication, documentation and organisational skills.
- A current Australian Driver Licence and willingness to travel within our service areas.
Experience with Support at Home and working with culturally and linguistically diverse (CALD) communities will be highly regarded.
