About the role
The Pharmacy Department provides services seven days a week and the position is required to participate in departmental rosters, including after-hours and weekend and on call services and may be called on to work in locations across Metro North Hospital and Health Service (MNHHS) as well as the Royal Brisbane and Women's Hospital (RBWH) campus.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
Provide clinical pharmacy services using experience and expertise in multiple areas within the pharmacy team (subject to rotation), including:
delivery of efficient and effective medicines distribution/dispensing to both inpatients and outpatients;
undertake medication history interviews upon admission, medication reviews, and formulation of pharmaceutical care plans including provision of high level assistance to staff of other health profession disciplines;
participate in discharge planning including meetings, ward visits and discharge medication counselling, and continuity of care liaison with community health providers;
provide accurate verbal and/or written drug information and education/training to medical, nursing and allied health staff and patients/carers;
undertake drug therapy monitoring and participate in medicines usage evaluations and collection of key performance indicator data.
