As a key member of the ATSICHS Brisbane team, the ATSICHS Kin Connector & Family Support Specialist program is a unique initiative that works to identify kin placement pathways for children and young people residing in the child safety residential OOHC system. The aim of the program is to re-unify family under safe and thriving conditions. The program is not only excluded to kin placement options but also places a great emphasis on family reunification with biological parents, when safe to do so. Coordinated and culturally safe support and interventions are provided to families. By offering a comprehensive mix of services tailored to family and community needs, we aim to build family capacity, strengthen community connections, and safeguard the wellbeing of children.
Duties
Effectively engage clients (extended family and kin of child) through cold-call home-based visits. Turn initial ‘kin engagement’ home visit into ongoing consent and interest in becoming a Kin Carer.
While completing family engagement activities, ask to identify other strong family members that could possibly become a kin carer. Actively seek contact details and relationship connection with child and broader family.
Engage the child and/or lead Residential Care Worker in determining the views and wishes of family placement options with family, as directed by line Manager. Utilise the Narrative Assessment Tools to assess barriers and wishes.
Assist in the assessment of carers, including initial screening, Blue Card submission, and PAC assessments. Advocate and support families when being assessed externally.
Develop a comprehensive Transition Plan (Case Plan) that explicitly addresses all concerns outlined in the (1) referral, (2) program assessment tool, (3) Transition NAT tools, (4) Child Assessment, and (5) Carer Assessment.
Deliver case managed services according to the program model of care, ensuring minimum case load requirements are maintained to ensure equitable access to services for community.
Maintain accurate and detailed case notes of all client interactions, ensuring case notes are written from the perspective of clients in their voice.
Ensure clients complete the ‘Have Your Say’ client feedback tool and submit. Act on the client feedback to make continuous improvements to your practice and approach with clients.
As per MOBnet Performance module, participate in monthly prescheduled supervision with direct line Manager. Ensure Supervision tasks are the priority in work completion. Ensure all associated monthly supervision tasks in MOBnet are completed (two-way feedback, team culture score etc).

Base Salary of $105,673 per annum (Based on qualification and experience).

Skills and Experiences:
Required
Degree Qualification+ Moderate Experience: Bachelor's degree in a relevant social services field with at least 4 years case management experience in family support, child protection, youth support, housing or related field.
OR
Diploma + High Experience: Diploma in relevant Community/Services field with at least 6 years in a case management role in family support, child protection, youth support, housing or related field.
OR
Nil Qualifications + High Experience + Strong Cultural Engagement: At least 6 years case management experience in family support, child protection, youth, housing or related field. A commitment to complete Diploma (funded by ATSICHS) while working. And identify as Aboriginal and/or Torres Strait Islander (with confirmation proof) OR non-indigenous and having worked for an Aboriginal Community Controlled Organisation for at least 4 years.
And
- Exceptional case management skills, including grammatical writing ability, personal organisation and time management, and execution of services according to the model of care phases of work.
- Demonstrated ability to create new social work theoretical frameworks of practice, aligned with ATSICHS values. Ability to coach other staff on client interventions that build client motivation, insight and acceptance, self-awareness, decision making (agency), and sustainable behaviour change.
- Ability to apply technical knowledge of relevant legislation (child protection/housing/youth justice etc). Ability to critique/ and problem solve legislative grey zones, while managing organisational and child/client harm risks.
- Influence decisions of external stakeholders, including Child Safety, Youth Justice, Schools, and Hospitals. Whole-of-system Internal Integration: Ability to connect through systems panel coordination numerous internal ATSICHS programs and services to activate wrap-around care for clients, trading-off and negotiating resources across the organisation and between other teams to achieve client impact.
