Your new role:
As the First Nations Nurse Navigator, you will provide advanced clinical leadership, care coordination and advocacy across the patient journey. Working across services and healthcare settings, you'll help reduce barriers, strengthen connections and ensure care is responsive to the needs, goals and cultural context of First Nations patients and their families. You will:
Coordinate person-centred care across primary, secondary and tertiary healthcare settings.
Build trusted relationships with patients, families, carers, healthcare teams and community partners.
Advocate for culturally safe care and help reduce fragmentation and barriers within the healthcare system.
Work collaboratively with the Aboriginal and/or Torres Strait Islander Health Directorate, Cultural Coordination Team, IUIH, Mob-Link, local Elders and community organisations.

About you:
You are an experienced Registered Nurse who brings advanced clinical capability, strong communication skills and a genuine commitment to improving health outcomes for First Nations peoples. You're confident working autonomously while building strong partnerships across multidisciplinary teams, healthcare services and community. You'll bring:
The ability to navigate complex healthcare systems and connect patients with appropriate services and supports.
Experience working collaboratively with patients, families, multidisciplinary teams and community organisations.
An understanding of the social determinants of health and the principles of Primary Health Care.
A relevant postgraduate qualification, or enrolment in postgraduate study, and previous experience in Aboriginal and Torres Strait Islander health will be highly regarded.

Logan Hospital is the major health centre for one of the fastest growing areas in Queensland. We offer a range of services, including medical, rehabilitation, maternity, surgical and paediatric care.
We offer our staff and students extensive support and training in most medical specialties. As a teaching hospital, we help train future health care professionals and participate in research through strong partnerships with Queensland's leading medical schools, universities, and TAFE Queensland. Our services will continue to grow as we build bigger and better facilities to support the community. By 2027, we'll have more beds and 5 more birthing suites as part of the Logan Hospital expansion.
