The Transition and Post Care Support Officer's key responsibilities are:
- Provide services based on the principles of the Strengthening Families Protecting Children Framework for Practice and the core elements of the Aboriginal and Torres Strait Islander Child Placement Principle.- Involve the young person in goal setting and person-centred planning while maintaining and supporting their connection to family, culture and community.- Organise and facilitate stakeholder meetings.- Organise the provision of specialist services that address high risk and complex behaviours and assist with the development of independent living skills including adaptive skills aimed to improve maintaining tenancies and co tenancies.- Consult and develop relationships with government and non-government agencies including Queensland Health, Department of Housing, Local Government, Planning and Public Works and the National Disability Insurance Agency to ensure efficient, effective and financially supported service delivery.- Develop, implement, maintain and review where necessary information systems, data bases and procedures relevant to the service.- Prepare documentation relevant to the service including letters, reports, meeting minutes, PowerPoint presentations and associated written communications.- Ensure program compliance and monitoring in terms of relevant human resource, financial management and other standards, policies and legislation relevant to the operations of the Department.- Enhance the local regional community by actively practicing and promoting excellence in service delivery and identifying opportunities for improving regional client service.This work is licensed under a Creative Commons Attribution 3.0 Australia License.
