Join Our Team as a Social Worker – Mental Health & General Wards
We are seeking compassionate and skilled Social Workers to join our dedicated team on a permanent basis, with both full-time and part-time opportunities available and a fixed-term maternity leave backfill position until 23 August 2027. In this crucial role, you will provide essential support to patients across our Mental Health and General Wards, as well as their families and carers.
About the Role:
As a Social Worker, you will play a vital role in enhancing the well-being and recovery journey of our patients. You will provide comprehensive psychosocial assessment, intervention, and support to individuals and families navigating complex health challenges within both mental health and general medical settings. Your responsibilities will include:

Utilising a variety of Social Work interventions to address patient and family needs.

Providing support and counselling to patients, their families, and carers.

Developing and implementing discharge plans that address the social, emotional, and practical needs of patients.

Connecting patients and families with appropriate community resources and support services, leveraging your knowledge of available networks.

Collaborating effectively within a multi-disciplinary team to ensure holistic patient care.

Exercising initiative and self-motivation while working both independently with minimal supervision and as a valued team member.
About You:
You are a dedicated Social Worker with a strong commitment to patient-centred care and ethical practice. We are looking for individuals who possess:

A Bachelor of Social Work (or equivalent) qualification.

Eligibility for full membership of the Australian Association of Social Workers (AASW).

Demonstrated experience in medical Social Work practice.

Well-developed skills in:

Problem-solving and ethical decision-making.

Organisation and time management.
-
Communication (written and verbal).

The ability to work effectively within a multi-disciplinary team environment.
