You'll be part of a close-knit team that values collaboration, compassion and purpose. Working across our South Melbourne office and Lakehouse Residences, you'll partner closely with the Program Coordinator to build meaningful relationships with residents and support them to achieve lasting housing and wellbeing outcomes.
As a Case Manager, you will provide holistic case management to women aged 40+ living at Lakehouse Residences, a transitional housing program in South Melbourne. The program supports women to settle and stabilise, build independence and economic participation, and successfully transition into long-term housing in the community.
Connect residents with housing, health, employment and community services, coordinating referrals and working collaboratively with external agencies to achieve positive outcomes.
