As demand for disability and community support services continues to increase, CoreAssist requires an experienced Community Worker to coordinate participant supports, facilitate community engagement, promote independence, and ensure participants receive high-quality, person-centred services that enable them to achieve their personal goals.
About the Role
The Community Worker is responsible for assessing participant needs, developing and implementing individual support strategies, coordinating access to community resources, monitoring participant progress, and assisting individuals living with disabilities to maximise independence and community participation.
Working closely with participants, families, allied health professionals, support coordinators, healthcare providers, and external agencies, the Community Worker plays an important role in delivering holistic support that promotes inclusion, wellbeing, personal development, and positive participant outcomes consistent with CoreAssist's person-centred service model.
The position requires excellent communication, interpersonal, organisational, and problem-solving skills together with a genuine commitment to improving the lives of people living with disabilities.
Key Responsibilities
- Conduct participant interviews and comprehensive needs assessments to identify individual goals, strengths, risks, and support requirements.
- Develop, implement, monitor, and regularly review individual support plans consistent with participant goals and NDIS plans.
- Support participants to develop independent living skills, social skills, confidence, and community participation.
- Assist participants to access healthcare, allied health, housing, education, employment, transport, recreational, and other community support services.
- Facilitate participant involvement in community activities, life skills programs, recreational programs, and social inclusion initiatives.
- Monitor participant wellbeing, evaluate service outcomes, maintain accurate case notes, and prepare progress reports.
- Provide advocacy, information, guidance, and practical assistance to participants and their families regarding available community services and supports.
- Liaise with support coordinators, allied health professionals, healthcare providers, government agencies, carers, and community organisations to coordinate integrated service delivery.
- Identify barriers affecting participant independence and implement appropriate support strategies to promote positive outcomes.
- Maintain confidential participant records, service documentation, incident reports, and case management records in accordance with NDIS Practice Standards and organisational policies.
- Promote participant rights, dignity, choice, privacy, cultural inclusion, and person-centred practice at all times.
- Participate in multidisciplinary meetings, quality improvement initiatives, and ongoing professional development.
- Assist CoreAssist in maintaining compliance with NDIS Practice Standards, Work Health and Safety legislation, Child Safe requirements, and organisational policies.
- Support continuous improvement initiatives that enhance participant satisfaction, service quality, and organisational performance.
Requirements
- Australian citizens and permanent residents are encouraged to apply.
- AQF Associate Degree, Advanced Diploma or Diploma in Community Services, Disability, Human Services, Welfare, Individual Support, or a closely related field, or at least three years of relevant experience may substitute for formal qualifications where applicable (ANZSCO Skill Level 2).
- Demonstrated experience working within disability services, community services, aged care, or NDIS-funded programs.
- Strong understanding of person-centred practice, participant rights, safeguarding principles, and community service delivery.
- Sound knowledge of the National Disability Insurance Scheme (NDIS), disability support services, community resources, and participant-centred care.
- Excellent communication, interpersonal, and relationship-building skills.
- Strong organisational, problem-solving, and case coordination abilities.
- Ability to maintain accurate participant records and prepare professional reports.
- Compassionate, empathetic, reliable, and participant-focused approach to service delivery.
- Ability to work independently and collaboratively within multidisciplinary teams.
- Current NDIS Worker Screening Check, National Police Check, Working with Children Check (where applicable), and Current First Aid/CPR certification or willingness to obtain.
- Current Australian Driver Licence is desirable.
