Support Coordinator (Fixed-Term Contract – Wednesday to Friday)

Assigned Guardian Angel is seeking an experienced and compassionate Support Coordinator to join our team on a fixed-term contract to provide leave coverage.
This position is three days per week (Wednesday, Thursday and Friday - 9am-4:30pm) for the duration of the contract.
About the Role
As a Support Coordinator, you will work closely with NDIS participants, their families, service providers and other stakeholders to ensure participants receive high-quality supports and achieve their goals.
Key Responsibilities
- Coordinate and implement NDIS plans.
- Liaise with participants, families, service providers and allied health professionals.
- Assist participants to access appropriate supports and services.
- Monitor budgets and service utilisation.
- Maintain accurate case notes and participant records.
- Prepare reports and documentation as required.
- Advocate for participants and support them to achieve their goals.
- Work collaboratively with the wider Support Coordination team.
About You
To be successful in this role, you will have:
- Previous experience as an NDIS Support Coordinator.
- Sound knowledge of the NDIS and relevant legislation.
- Excellent communication and relationship-building skills.
- Strong organisational and time management skills.
- Ability to work independently and manage competing priorities.
- Current NDIS Worker Screening Check.
- Current Working with Children Check (if applicable).
- Current National Police Check.
- Current Driver Licence and reliable vehicle.
