We are currently recruiting an experienced Clinical Care Coordinator to join a well-established aged care and disability care provider located in Melbourne’s South East.
This medium-sized residential facility is recognised for its strong community presence, supportive leadership team, and commitment to delivering high-quality, person-centred care. This is a key leadership role responsible for supporting the Facility Manager in overseeing clinical operations, ensuring compliance with aged care standards, and maintaining exceptional quality care outcomes across the home.
About the Role
As the Clinical Care Coordinator, you will provide hands-on clinical leadership and oversee the day-to-day coordination of care services within the facility. You will play an integral role in supporting and mentoring staff while ensuring compliance with accreditation standards and continuous improvement initiatives.

About You
- Current AHPRA registration as a Registered Nurse with no restrictions
- Minimum 2–3 years’ experience within residential aged care management
- Strong understanding of aged care legislation, accreditation, and quality standards
- Strong knowledge of NDIS standards
- Experience managing high care and complex care needs
- Excellent leadership, communication, and organisational skills
- A proactive, hands-on, and team-focused approach
