About the role:
The Pathways program is a voluntary, short-term program which aims to create pathways out of homelessness for people living with mental health issues and complex needs. Pathways aims to prevent ‘discharge to homelessness’ for individuals exiting short term bed based psychiatric settings as a priority. Referrals are received from a variety of sources.
The Housing Support Worker provides person-centred services to our customers to maintain their wellbeing, explore opportunities and work towards the agreed outcomes that are important to the customer. As the key
point of contact for customers in the Pathways program, the worker will provide efficient and effective day to day service delivery while facilitating the building of both formal and informal connection building that is relevant to the customer’s needs.
The provision of services is delivered using the ‘Housing First’ principals ensuring the application of the Human Services Standards and relevant legislation.
Key responsibilities include:
- Providing 1:1 support to designated clients in the outreach programs to create pathways from homelessness to secure long term housing and support for people who have been admitted to an acute care facility and other beds from clinical services within the Mid-West Area Mental Health catchment.
- Support customers to achieve their goals through person-centred approaches, ensuring compliance with relevant Industry Standards and established Breakthru policies, procedures and guidelines and within the principles of psycho-social rehabilitation.
- Actively encourage and engage customers to build their independence and ensure their needs are met including personal care, living skills, social and other supports.
- Undertakes intake assessments to determine their eligibility and suitability for the Pathways Program based on the established criteria.
- Developing plans in consultation with the customer and relevant stakeholders within 4 weeks of the initial appointment.
- Plan reviews conducted on a regular basis and reviews key areas including progress towards the established goals or milestones, and to determine if the current support structures in place are appropriate.
- Applying for the use of brokerage funds, that can be used to assist pathways customers in housing crises and expenses that may be identified in plans as investments to prevent homelessness in the future.
- Advocating of behalf of clients in relation to their housing needs.
About you:
- Responsible for planning and organising own work.
- Demonstrate understanding of diversity awareness.
- Meet expectations of quality, quantity and timeliness of supports within the agreed supports, plan or budget.
- Work collaboratively with team members, sharing knowledge and providing guidance to less experienced team members or volunteers.
- Demonstrate an ability to resolve conflict
- Maintain knowledge of supports and services available both internally and externally, demonstrating flexibility and suggesting alternative solutions or making necessary referrals.
- Identifies warning signs, risks and solutions for immediate work problems relating to both customers and other employees, as appropriate.
- Shows a commitment to ongoing skills development and participates in relevant training and supervision
Skills & Experience:
- Well developed oral and written communication with customers and other members of the public.
- Ability to work collaboratively with the customers treatment team and support networks
- Understanding of basic computing concepts, systems and software.
- Ability to adhere to and apply practices and techniques relevant to Breakthru.
- Ability to apply guidance from procedures and guidelines.
- Apply person centred practice to all interactions with customers
