Location: Moe, Victoria
Disability Connect Victoria is seeking a reliable and experienced Disability Support Worker to join our team in Moe on a permanent part-time basis.
This role involves supporting a participant with daily living activities while promoting their independence, choice and wellbeing. The successful applicant will need to be confident providing manual handling support and managing health-related needs in line with the participant’s individual support plans.
- Thursday: 3:00 pm to 7:00 am, including a sleepover
- Saturday: 7:00 am to 3:00 pm
- Sunday: 7:00 am to 3:00 pm
Applicants must be available to work the regular roster listed above.
Your responsibilities will include:

Supporting the participant with daily living activities
- Providing domestic assistance and meal preparation
- Completing manual handling using a hoist and sling
- Supporting diabetes management in line with management plans
- Supporting medication administration
- Monitoring and supporting asthma management
- Completing accurate progress notes and documentation
- Working collaboratively with the participant’s support team
- Providing sleepover support
A private vehicle is not required for this position.
To be successful, you will have:

Experience using hoists and slings
- Experience supporting people with diabetes
- Knowledge of asthma management
- Medication administration experience
- Safe manual handling skills
- Good cooking and household management skills
- Strong communication skills
- The ability to work as part of a team
- Initiative and the ability to work independently
- A person-centred and respectful approach
- Certificate III in Individual Support, Disability or a related qualification
- Current NDIS Worker Screening Check
- Current Working with Children Check
- Current First Aid and CPR certificates
- NDIS Worker Orientation Module
- Evidence of Australian working rights
