About Us:
The Cairns and Hinterland Mental Health and Alcohol, Other Drugs Service (MHAODS) is responsible for delivering Mental Health and AOD Services throughout the Cairns and Hinterland Hospital and Health Service and for supporting other mental health and AODS in the surrounding Hospital and Health Services of Cape York, Torres Strait Northern Peninsula Area. Its primary objective is to ensure access to a comprehensive range of treatment and support services, clinical treatment, rehabilitation, disability support, as well as early intervention, and mental health prevention and promotion.
About the Role:
Provide high level, independent clinical assessment, formulation and intervention for prisoners with complex and severe mental health needs transitioning between custody and community settings, ensuring evidence-based and recovery-oriented care.
Lead and coordinate transition planning for individuals with complex mental health needs, facilitating safe, timely and clinically appropriate continuity of care between custodial mental health services, community mental health teams and external service providers.
Deliver clinical consultation and advice to multidisciplinary team members and external stakeholders regarding complex mental health presentations, risk management and transition planning.
Develop, implement and review individualised transition and discharge plans, ensuring integration of mental health, alcohol and other drug, housing, psychosocial and primary health supports.
