Senior Pharmacist – Quality Use of Medicines
- Ongoing, Full Time Role - 80 Hours per Fortnight with a Monthly ADO
- Parkville location, with multiple transport options
- Up to $123,905 base (prorated for part time) + salary packaging + super + 5 weeks annual leave

About the Team
The Pharmacy department at Peter Mac provides a comprehensive patient-focused service, incorporating all facets of innovative clinical pharmacy practice. Specialised areas include antimicrobial stewardship, quality use of medicines, pharmacogenomics, clinical cancer informatics, medicines information, and clinical education. The department has an ongoing commitment to practice-based research and offers opportunities for involvement to all staff.
About the Role
The Quality Use of Medicines (QUM) Pharmacist (1.0 FTE) is responsible for oversight and coordination of all medication safety in the organization in accordance with the National Standards for Safety and Quality in Healthcare. This role includes monitoring prescribing trends, performing audits, and utilising and evaluating data to inform and drive medication safety and quality improvement initiatives. The QUM Pharmacist works collaboratively with Quality and Safety and Precinct Partners, and represents the pharmacy department within the healthcare service.
About you
We are seeking an enthusiastic, experienced and motivated pharmacist to lead our Quality Use of Medicines (QUM) program within a collaborative team of medicines stewardship pharmacists, quality leaders and multidisciplinary clinicians.
You will have excellent communication and interpersonal skills, with the ability to build strong relationships across the organisation, influence practice, and lead initiatives that improve medication safety, governance and the quality use of medicines.
You’ll bring with you:
- AHPRA registration, or be eligible for registration.
- Well-developed organisation and prioritisation skills.
- Demonstrated understanding of quality use of medicines principles and medication safety.
- Previous experience in oncology pharmacy practice is highly desirable.

Skills
Essential Requirements
Qualified Pharmacist, Registered with Australian Health Practitioner Registration Agency (AHPRA)
A minimum of 5 years experience in hospital pharmacy practice with demonstrated development in a specialised area of practice
Post-registration qualification (e.g. diploma, masters, doctorate) or equivalent competency-based credentialing demonstrating advanced practice capability
Demonstrated understanding of quality use of medicines principles and medication safety
Demonstrated active contribution to quality improvement or practice development, with evidence of increasing responsibility in design, implementation, or evaluation of initiatives
Demonstrated competency in pharmacy clinical workflows with significant QUM implications, including high risk medicines (i.e. chemotherapy verification), inpatient services, day therapy, and outpatient pharmacy.
Evidence of consistent and high-quality professional practice, demonstrating ability to act as a point of reference within area of practice
Evidence of consistent engagement and progression in professional development
Demonstrated experience in supervision and development of others, including students, interns, or pharmacists, contributing to learning and performance within the team.
Demonstrated contribution to education and training, including delivery of teaching and development of educational activities or resources.
Demonstrated well-developed communication, collaboration, stakeholder engagement, prioritisation, and organisational skills within a multidisciplinary healthcare environment.
Desirable Requirements
Completion of relevant research governance and clinical research training (e.g. ACTEC, Research Integrity, Good Clinical Practice [GCP]) to support safe, ethical and high-quality audit, quality improvement and research activities.
Demonstrated capability to lead or manage a team, program, or project, or undertake delegated leadership responsibilities.
Demonstrates ongoing and active contribution to research, quality improvement, or practice development.
Demonstrated engagement in presentation of work, including contribution as a presenter at internal and/or external forums.

1. Operational Management
Attend weekly medication safety huddles to provide anticoagulation stewardship expertise and support review and escalation of high-risk incidents.
Lead monthly anticoagulation stewardship governance meetings, reporting incidents, initiatives and emerging risks to support service improvement.
Serve as secretary to the multidisciplinary anticoagulation committee, coordinating meetings, documentation and follow-up of governance actions.
Partner with pharmacy stewardship programs and QUM to deliver anticoagulation stewardship aligned with medication safety priorities.
Design, implement and evaluate anticoagulation stewardship policies supporting NSQHS Standard 4.
Develop and implement accreditation plans supporting NSQHS Standard 4, focusing on anticoagulant safety.
Liaise with Quality and Safety Partners to address incidents and develop evidence-based improvements for anticoagulant use.
Review clinical incidents to identify trends and risks involving anticoagulants and other high-risk medicines.
Participate in serious incident reviews and root cause analyses, recommending system improvements for anticoagulation safety.
Coordinate anticoagulation stewardship audits and surveys to monitor compliance and identify improvement opportunities.
Coordinate multidisciplinary meetings to complete anticoagulation medication safety self-assessments and stewardship reviews.
Collect and report anticoagulation stewardship indicators to support program monitoring and governance.
2. Clinical Expertise / Skills
Demonstrate advanced clinical pharmacy and anticoagulation expertise across diverse patient populations, particularly in oncology and acute care.
Develop and review clinical procedures, guidelines and frameworks to support safe, effective anticoagulant use.

3. Practice Improvement and Service Development
Liaise with Medication Safety pharmacists at The Royal Melbourne Hospital, Peter MacCallum Cancer Centre, Royal Children’s Hospital and Local Health Network partners to:
i. Align medication policies, guidelines, procedures and forms across Precinct Partners where possible.
ii. Support consistent design and delivery of medication management improvement projects.
iii. Support optimisation of medication management within the Electronic Medical Record (EMR).
iv. Identify and address medicine-related risks across the precinct.
v. Coordinate Precinct Medication Safety Committee activities.
vi. Coordinate Peter Mac Medication Safety Committee activities.
Work with Medical, Nursing and the Pharmacy and Therapeutics Advisory Committee to develop and implement a Quality Use of Medicines Program and associated projects.
Maintain a quality improvement focus and ensure compliance with National Standards for accreditation.
4. Research, Education and Training
Develop and deliver education for staff and pharmacy students on emerging QUM priorities.
Participate in research and quality improvement projects supporting quality use of medicines and medication safety.
Support clinical staff development through continuing education, conference attendance and presentations.
5. Information and Data Management
Maintain accurate records in accordance with hospital policies, procedures and guidelines.
Develop and maintain medication safety dashboards and reports, presenting findings to stakeholders.
Ensure documentation and data submissions comply with statutory and hospital requirements.
Coordinate the department audit schedule, providing regular reports and escalating issues where required.
6. Stakeholder Relationships
Develop and maintain effective relationships with key internal and external stakeholders.
7. Planning
Contribute to departmental strategic planning.
8. Advocacy
Promote patient safety through safe medicine use, medication error reduction and best practice.
Promote the Pharmacy Department and represent the profession as required.
Attend and contribute to Adverse Event Committee, Precinct Medication Safety Advisory Group (EMR), pharmacy clinical and research meetings, and other relevant forums.
9. Communication
Communicate with empathy and professionalism when engaging with patients, families and staff.
Facilitate communication between pharmacy, medical and nursing teams to support medication safety.
Provide medication safety and QUM updates to relevant committees.
Handover planned work, work in progress and significant issues to colleagues.
Collaborate with Precinct partners (RMH, RCH and RWH), Victorian Therapeutic Advisory Group (VicTAG) and other cancer centres on statewide QUM initiatives and best practice.
