The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).
We are inviting experienced Social Workers to join a dedicated team committed to improving, maintaining and promoting the health and wellbeing of Aboriginal children, families and communities. This rewarding role supports the delivery of the Building Strong Foundations (BSF) program and the Aboriginal Maternal and Infant Health Service (AMIHS), providing specialist social work services that is culturally responsive and family-centred care through respectful, trusting and enduring relationships.
As a Social Worker, you will ensure the provision of comprehensive psychosocial assessments, care coordination and evidence-based therapeutic interventions. You will play a key role in supporting Aboriginal children aged 0–5 years, expectant mothers and families, while contributing to service planning, quality improvement, evaluation and reporting. The position also provides clinical supervision, professional leadership and specialist consultation to psychosocial clinicians, helping to ensure the delivery of high-quality, culturally safe care across the service.
