What your day could look like
You'll be part of a service that helps older people stay well, maintain independence and receive care in the setting that best meets their needs. You'll combine advanced clinical practice with leadership, working with a multidisciplinary team that shares your commitment to compassionate, person-centred care. Every day is an opportunity to make a meaningful difference for patients, families and the community.
Every day, you'll contribute by:
Guiding complex care decisions that help older people achieve safer, better health outcomes
Building trusted relationships with patients, families and healthcare professionals
Mentoring and supporting colleagues, helping create a positive team culture
Championing quality improvement initiatives that enhance patient experiences, strengthen safety and shape the future of care delivery
Let's talk about you!
You see the person behind the presentation. You combine clinical expertise with empathy, communicate with confidence, and know how to bring people together around a shared goal. You thrive in environments where no two days are the same and find fulfilment in helping older people remain as independent, healthy and connected as possible.
This role offers more than professional growth. It's an opportunity to be part of a connected regional community, work alongside passionate healthcare professionals and build a career that continues to evolve through learning, leadership and development opportunities.
Here's what will help you thrive in this role:
Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia
Strong clinical assessment, decision-making and care coordination skills
Experience working collaboratively within multidisciplinary healthcare teams
A commitment to delivering safe, high-quality, person-centred care and supporting continuous improvement
You'll stand out from the crowd if you have postgraduate qualifications relevant to aged care, rehabilitation or community health and/or experience in rehabilitation, hospital substitution or community-based care services.
