About the Role
We are seeking an experienced and passionate Support Coordinator to join our growing team.
You will work closely with NDIS participants to implement their plans, coordinate supports, build capacity, and connect them with services that help them achieve their goals. This role involves participant meetings, home visits, stakeholder collaboration, advocacy, and maintaining accurate documentation.
Key Responsibilities
- Coordinate and implement NDIS plans.
- Support participants to build independence and exercise choice and control.
- Manage participant budgets and coordinate appropriate supports.
- Liaise with participants, families, providers, and allied health professionals.
- Conduct participant reviews, home visits, and community appointments.
- Maintain accurate case notes and reports.
- Advocate for participants and assist with complex situations.
- Ensure compliance with NDIS Practice Standards and organisational policies.
About You
You will have:
- Minimum 2 years' experience in Support Coordination or a similar NDIS role.
- Strong understanding of the NDIS and person-centred practice.
- Excellent communication, organisation, and problem-solving skills.
- Ability to work independently and as part of a collaborative team.
- A genuine passion for supporting people with disability.
Essential Requirements
- Current NDIS Worker Screening Check.
- Working with Children Check.
- National Police Clearance.
- Current Australian Driver's Licence and reliable vehicle.
- NDIS Worker Orientation Module Certificate.
- Relevant qualification in Community Services, Disability, Social Work, or a related field (desirable).
