The RACE (Rapid Access Care of the Elderly) team provides specialised care to older adults, supporting patients and their families through complex medical, social and emotional challenges.
You will be part of a supportive Allied Health environment that values collaboration, innovation and professional growth, working alongside medical, nursing and allied health professionals to achieve positive outcomes for patients and their support networks.

What you'll be doing
As a Level 1-2 Social Worker, you will provide high-quality social work services to patients, families and carers receiving care through Geriatric Medicine services. You will apply your clinical knowledge and professional judgement to support patients experiencing complex psychosocial circumstances and changing health needs.
- Conduct comprehensive psychosocial assessments and provide counselling, advocacy, crisis intervention and appropriate referrals to community supports
- Support patients and families navigating complex issues including adjustment to illness, elder abuse, guardianship matters, domestic violence and sudden changes in health status
- Collaborate with multidisciplinary teams through ward rounds, care meetings and clinical discussions to support holistic patient outcomes
- Maintain high standards of clinical documentation, data reporting and professional practice in line with NSW Health and Social Work guidelines
- Participate in supervision, education, quality improvement activities and support Social Work student clinical placements where required

Essential Criteria:
- Degree in Social Work with eligibility for membership of the Australian Association of Social Work (AASW). Qualification: Bachelor of Social Work
Selection Criteria:
1. Demonstrated understanding of issues associated with ageing, age related illnesses, rehabilitation, accommodation placements, guardianship and end of life care.
2. Unrestricted NSW Driver’s Licence and ability to participate in the After Hours Crisis On-Call Social Work Service.
3. A good knowledge of community resources and experience liaising with community organisations
4. Demonstrated clinical experience or understanding of grief and loss counselling, crisis intervention, discharge planning and working with complex family dynamics.
5. Demonstrated understanding of issues associated with working with culturally and Linguistically Diverse client group.
6. Demonstrated well-developed time management skills with excellent written and verbal communication, including report writing skills and high level computer literacy.
7. Demonstrated ability to work with minimal supervision and as part of a multidisciplinary team
8. Commitment to quality improvement and evidence based practice and a commitment to ongoing clinical and professional learning
