About the Role
Pacific Home Care Solutions (PHCS) is looking for a confident and proactive Community Engagement & Programs Officer to join our Melbourne team.
The main focus of this role is to build PHCS’s connections within the disability and community sector and help generate genuine participant referral opportunities.
You will spend time connecting with Support Coordinators, Psychosocial Recovery Coaches, allied health professionals, community organisations and other referral networks, introducing PHCS services and developing ongoing professional relationships.
Alongside community engagement, you will also have the opportunity to help organise and lead group activities, day trips and community participation programs for people with disability.
This is a KPI-based position, so we are looking for someone who is comfortable networking, approaching new organisations, following up opportunities and working towards reasonable community engagement and referral targets.
Key Responsibilities
- Build relationships with Support Coordinators, Recovery Coaches, allied health professionals and community organisations.
- Promote PHCS services and develop genuine participant referral opportunities.
- Visit relevant organisations and attend networking/community events.
- Follow up prospective referrals and stakeholder relationships.
- Maintain records of community engagement activities and referral opportunities.
- Help develop and coordinate group activities, day trips and social/community programs.
- Lead or assist with participant group activities as required.
- Develop new ideas to increase participant and community engagement.
- Provide management with regular updates against agreed KPIs.
- Represent PHCS professionally and maintain participant choice, privacy and professional boundaries.
About You

- Have existing disability/community-sector networks – highly regarded.
Requirements
- NDIS Worker Screening Clearance
- National Police Check
- Current First Aid & CPR
- Current Australian driver’s licence
- Reliable vehicle and appropriate insurance where required
- Relevant community services/disability qualifications are desirable

Pay: $45.00 – $55.00 per hour
