About the Role
Reporting to the Care Manager, you will coordinate the home's clinical assessment and care planning program, ensuring assessments, documentation and care plans remain accurate, current and aligned with each resident's individual needs.
Working closely with Registered Nurses and the multidisciplinary team, you will contribute to clinical governance, continuous improvement and quality outcomes while helping to strengthen assessment and documentation practices across the home.
Key Accountabilities
Care Planning

Coordinate the Care Plan review program in partnership with the Care Manager.

Lead care planning for new admissions and ensure comprehensive assessments are completed.

Review care plans regularly to ensure they reflect residents' changing clinical needs.

Monitor changes in resident care needs to identify opportunities for reassessment and continuous improvement.
Clinical Leadership and Quality

Contribute to clinical governance and quality improvement initiatives.

Promote best-practice assessment, documentation and care planning.

Assist in developing education initiatives that strengthen clinical capability across the nursing team.

Maintain high standards of clinical documentation and compliance with legislative and organisational requirements.
Stakeholder Engagement

Build effective working relationships with residents, families, medical practitioners and the multidisciplinary team.

Work in partnership with internal stakeholders to achieve positive clinical outcomes and a consistent resident experience.
What You'll Bring
You are a Registered Nurse who enjoys improving clinical practice, mentoring others and contributing to quality resident care. To be successful in this role, you will have:
Essential

Current registration as a Registered Nurse.

Minimum two years' clinical experience, preferably in aged care, palliative care or geriatrics.

Sound knowledge of clinical assessment, care planning and documentation.

Knowledge of the Aged Care Quality Standards.

Strong written, verbal and stakeholder communication skills.
-
Well-developed organisational and time management skills.
-
Confidence using clinical software and Microsoft Office applications.

Current National Police Check.
-
Current Australian working rights.
-
Current Influenza and COVID-19 vaccinations in accordance with applicable government or organisational requirements.
Desirable
-
Experience coordinating care planning within residential aged care.

Experience participating in clinical governance or quality improvement activities.

Leadership or management qualifications.

Experience with AN-ACC documentation and assessment.
