About the Role:
Acute Care Evaluation (GRACE) Service, where you will play a vital role in delivering responsive, patient-centred care to older people. In this dynamic position, you will work closely with Residential Aged Care Home residents, their families and carers, General Practitioners, Emergency Department staff, inpatient teams across North Canberra Hospital and Canberra Hospital, and the ACT Ambulance Service to improve outcomes for acutely unwell residents.
The primary focus of this role is to provide timely assessment and expert clinical management aimed at preventing avoidable presentations to public Emergency Departments. Reporting to the Level 3.2 Clinical Nurse Consultant, you will be part of an innovative model of care that supports residents in the most appropriate setting. The GRACE Service delivers a comprehensive approach, including outreach support and education for Residential Aged Care Home staff, acute clinical assessment and care provided directly in residents’ homes, a coordinated single-entry point for hospital transfers, and advocacy and case management across Emergency Department and inpatient settings, alongside ongoing post-discharge support to ensure continuity of care.
