About the Role
Whoever You Are is seeking an active, detail-oriented Support Worker Team Leader / Disability Services Officer to oversee direct care operations, staff supervision and participant services across South East Queensland and Northern New South Wales.
Reporting to our company headquarters, this position is a mobile, field-based role split across client settings in both South East QLD and Northern NSW. You will be responsible for participant intake, developing care plans, inducting and training direct care staff and ensuring 1:1 in-home and community care strictly meets NDIS Quality and Safeguards standards.
Key Duties & Responsibilities
- Participant Intake & Care Planning: Conduct intake assessments for new participants; design, implement and maintain individual NDIS care plans, risk assessments and behavioural support strategies.
- Staff Induction & Training: Induct, train, mentor and supervise direct care staff delivering 1:1 in-home and community-based support.
- Rostering & Operational Supervision: Manage daily shift scheduling, staff-participant matching, and workflow coordination across QLD and Northern NSW service sites.
- Direct Care & Field Supervision: Provide 1:1 direct community and at-home support to maintain operational continuity, model best-practice care, and monitor service quality in the field.
- Compliance & Documentation: Maintain accurate client records, draft incident reports, update care plans, and ensure documentation complies with NDIS audit standards.
- Cross-Border Travel: Travel regularly across client and staff locations in South East QLD and Northern NSW to conduct site visits and staff inductions.
Qualifications & Experience Required
- Education: Diploma or Bachelor’s degree in Psychology, Criminology, Social Work, Human Services, or a related discipline (or equivalent relevant operational experience).
- Experience: Demonstrated experience in disability support, with strong capabilities in care plan development, participant intake, and leading or guiding direct care staff.
- Proven Capability: supporting participants with complex physical disibilities, intellectual disabilities, and psychosocial/mental health conditions.
- Practical experience and current competency in medication administration and manual handling practices and equipment (e.g. hoists, moility aids, transfers).
- Clearances & Mobility:
- Active NDIS Worker Screening Check (or QLD Blue/Yellow Card equivalent).
- Current First Aid & CPR Certification.
- Valid Australian Driver’s License and willingness to travel across QLD / Northern NSW border regions.
