War Memorial Hospital (WMH) is a 3rd Schedule Aged Rehabilitation hospital, situated in Waverley, owned and operated by Uniting and part of the South Eastern Sydney Local Health District (SESLHD). Services are comprised of 24-hour inpatient care, Monday to Friday outpatient departments, iREAP Day Rehabilitation program, several specialist community teams inclusive of Transitional Aged Care, Geriatric Flying Squad, Young Onset Dementia, and day centre program, 24-hour accommodation service for regional guests and the full range of hospital support services. The Rehabilitation Clinical Nurse Consultant Grade 2 (CNC is a senior clinical leadership role at War Memorial Hospital, providing advanced consultancy, leadership and expert nursing practice across both inpatient aged rehabilitation (Morgan Unit) and the Transitional Aged Care Program (TACP).
This position leads clinical excellence across the full patient journey, from inpatient rehabilitation through to community-based transitional care, supporting complex care coordination, optimal functional outcomes and person-centred service delivery.
Working closely with the Nurse Unit Manager, TACP Manager and WMH Executive, the CNC2 plays a key role in clinical service development, workforce capability building, and the advancement of evidence-based practice across both services. The role provides high-level clinical guidance to multidisciplinary teams, drives quality improvement initiatives, and contributes to strategic planning, research and education aligned with ARNA competencies and NSW Health frameworks.
As an expert resource in aged rehabilitation and transitional care nursing, the CNC2 leads clinical supervision, education and service innovation, fostering a culture of excellence, accountability and continuous improvement. The position represents WMH at local and district forums and supports the development of integrated models of care across inpatient and community settings.

Qualifications:

Current Registration with AHPRA as a Registered Nurse with a minimum of 5 years full-time equivalent post-registration experience, including at least 3 years in aged rehabilitation, gerontology, community care or related specialty.

Approved postgraduate nursing qualifications relevant to Aged Care, Rehabilitation or Chronic/Complex Care or commitment to commence within 12 months.

Current Class C Driver’s Licence (required for community-based TACP activities).

Demonstrated advanced clinical leadership, expert clinical reasoning and the ability to deliver, evaluate and guide high-quality patient care across inpatient and community settings.

Proven experience in quality improvement, service development, research translation and the ability to lead multidisciplinary improvement initiatives.

Excellent interpersonal, written and verbal communication skills, including high-level computer literacy.

Demonstrated commitment to consumer participation, person-centred care, and engagement in service planning, evaluation and policy development.
