About the Opportunity
Join the Hunter Valley Transitional Aged Care Program (TACP) and become part of a dedicated multidisciplinary team committed to supporting older people as they transition safely from hospital back to their homes.
Our team of nursing and allied health professionals works collaboratively with clients and their families to deliver person-centred, goal-focused care that promotes independence, wellbeing and safety. Using an enablement approach, we provide restorative care and slow-stream therapy designed to help clients achieve their functional goals and remain living independently for as long as possible. This is a community-based role where you'll provide care through home visits across the Hunter Valley. The service operates Monday to Friday, 8:00am to 4:30pm, offering the opportunity to enjoy regular business hours with no shift work. From your first day, you'll receive a comprehensive orientation to the program, ongoing support from an experienced multidisciplinary team, and access to mandatory education and professional development opportunities to help you build confidence and succeed in the role.
What You Will Be Doing
As a valued member of the Hunter Valley TACP team, you will work closely with clients, their families and the multidisciplinary team to deliver high-quality, person-centred care within the community.
Your responsibilities will include:
- Supporting older people to achieve their rehabilitation and functional goals following discharge from hospital.
- Providing holistic nursing care and monitoring clients' health, wellbeing and progress.
- Conducting home visits and completing comprehensive assessments.
- Collaborating with allied health professionals to develop and implement individualised care plans.
- Educating and empowering clients and carers to maximise independence and safety at home.
- Maintaining accurate clinical documentation using NSW Health systems.
- Managing your daily workload independently while contributing positively to a collaborative team environment.
- Communicating effectively with clients, families and healthcare providers to ensure coordinated care.
About you
We're looking for a compassionate and motivated clinician who is passionate about supporting older people to live safely and independently in their own homes. You enjoy working autonomously while remaining connected to a supportive multidisciplinary team and have excellent communication, organisational and time management skills.

Experience working within a Transitional Aged Care Program, rehabilitation, community nursing or aged care setting is highly regarded but not essential. Knowledge of the Single Digital Patient Record (SDPR) is advantageous; however, full training and support will be provided.
Essential Requirements
- Current registration as a Registered Nurse with AHPRA.
- Current Class C Driver's Licence.
Willingness to travel within the service area to conduct home visits.
