Are You the Right Fit?
You are an Enrolled Nurse with knowledge and experience working with aged care and/or community populations and a strong interest in supporting older adults through reablement focused care. You bring sound clinical reasoning skills across a diverse range of clinical areas and are confident in assessing TACP clients within residential aged care facilities, contributing to holistic goal setting, developing client centred care and intervention plans, and supporting effective discharge planning. You work well within a team environment and value collaboration, working closely with a Registered Nurse and other members of the multidisciplinary team to deliver safe, evidence based and person centred care.
Please note – This role is NOT based in the inpatient hospital setting. The role is working for community health and is based in residential aged care facilities – it will include a wide range of tasks – eg. Transporting/escorting clients to medical appointments (where necessary). This is a permanent role for NSW health, temporary visa holders are NOT eligible.
Selection Criteria
To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below.
1. Current Enrolled Nurse (Division registration with the Nursing and Midwifery Board of Australia with no notations related to administration of medication.
2. Demonstrated Nursing knowledge and skills relevant to Community Nursing (e.g. skin assessments, risk identification, escalating clinical deterioration and so on).
3. Knowledge of and ability to work within a person-centered approach to nursing with a willingness to embrace change in the delivery of care.
4. Demonstrated ability to work independently with minimal supervision while effectively collaborating within a multidisciplinary team to achieve shared goals and deliver high-quality outcomes.
5. Applicant must possess an Australian C-Class driver's license (including P1 or P? Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
