What you'll be doing
This position will support patients accessing MNC Virtual Care through the provision of screening, flow management, check-in and general enquiries.
This position works with individuals and their families, health practitioners and communities to support patients and link them to health and other services. This position requires good skills in communication, identifying and solving barriers in access to care, collaborative problem solving and knowledge of referral to health services.
The primary purpose of MNC Virtual Care is to reduce preventable admissions and decrease length of hospital stay through Urgent Care service delivery, early intervention, and care navigation to people who have been identified as at risk of presentation and hospitalisation for low acuity conditions that can be safely managed over a virtual platform. This is not a remote working position.
About You
You are a compassionate and capable healthcare support professional with a strong foundation in health services and patient-centred care. Your qualifications and experience, whether through Health Services Assistance, Allied Health Assistance, undergraduate nursing or allied health training, have equipped you with the skills to provide safe, effective, and responsive care within a multidisciplinary healthcare environment. You communicate confidently and respectfully with patients, carers, and healthcare teams, ensuring individual care needs are understood and addressed. With strong written, verbal, and interpersonal communication skills, you are comfortable using a range of technology platforms and are enthusiastic about learning new systems and applications. You apply critical thinking and evidence-based principles to support quality care delivery, work effectively under supervision, and have a thorough understanding of your scope of practice, always maintaining professional boundaries. Reliable, adaptable, and committed to ongoing learning, you hold an unrestricted NSW driver's licence and are willing to travel as required to support patient care and service delivery.
