Permalink Support Services is a registered National Disability Insurance Scheme (NDIS) Provider, that offers Homecare & Community Care services to people with a disability.
Servicing in all regions of Melbourne, Permalink is rapidly growing and expanding its care services in the Northern, Western, Southern, and Eastern regions of Melbourne, supporting more than 500 people each year!
We are seeking a dedicated and compassionate CASUAL DISABILITY COORDINATOR & EMPLOYMENT SUPPORT WORKER to join our team either on 3, 4 or 5 days per week, 9:00AM-4:00 PM. The ideal candidate will have a genuine interest in improving the lives of individuals with disabilities and providing high-quality support services and 1:1 support to our participants.
Our Disability Support Workers & Coordinators contribute to the achievement of servicing people who require immediate access to services and ongoing rosters of care. Services include providing personal care, assistance with domestic tasks, and facilitating community access.
You will be hired directly by Permalink as an employee and not subcontracted. (TFN only, not ABN)
The DISABILITY COORDINATOR & EMPLOYMENT SUPPORT WORKER responsibilities will include and not limited to:
- Consistently attend scheduled weekly work sessions, ensure a positive participant experience, and comply with NDIS regulations.
- Responding to participant issues/incident reporting to ensure they are supported in a timely and professional manner.
- Liaising with Permalink participant rostering team, service specialists, participant plan nominees, and participant’s next-of-kin, to gather accurate details of the participant and their Support Plans.
- Understanding participant NDIS Plans, goals, and Care Plans, to ensure shift tasks are met.
- Coordinating placement students completing their Masters of Social Work.
- Attending to placement meetings and supervision meetings.
- Facilitating group supervision for Masters of Social Work students.
- Relaying crucial updates and alerts to the Rostering Team.
- Supporting an inclusive participant-centric culture and proactive approach.
- Contributing to the provision of relevant information and reporting.
- Contributing to the process of documentation and continuous improvement activities.
- Providing information to and participating in different care teams as appropriate.
We are seeking a highly driven and motivated individual with strong verbal and written communication and relationship-building skills. The right person for our team will possess strong problem-solving skills and someone who can exhibit a can-do attitude.
Please note that experience in the NDIS industry or any healthcare industry is a must!
Qualifications and experience:
-Must have a Masters of Social Work with a minimum 2 years post industry experience in Social Work or Case Management
-AASW Accrediation
-Must have evident experience in Disability Care and the NDIS sector
-Fluent and strong verbal and written communication
-NDIS Worker Screener clearance
-NDIS Worker Orientation Module Certificate
-Working with Children Check
-Police Check
-Updated First Aid & CPR
