Cafs is seeking motivated and compassionate Better Futures Practitioners to join our Youth Services team. We have opportunities available across SW4 and SW5 classifications, supporting young people with a care experience to build independence, strengthen connections and create positive futures.

As a Better Futures Practitioner, you'll work directly with young people transitioning from care, helping them develop the confidence, skills and connections needed for adulthood.
You will:
- Build strong therapeutic relationships with young people.
- Support goals across housing, education, employment, wellbeing and community connection.
- Advocate for young people and coordinate supports around them.
- Manage brokerage funding and referrals.
- Work collaboratively with carers, care teams and partner agencies.
- Deliver trauma-informed, strengths-based support that helps young people achieve meaningful outcomes.
SW4 positions focus on direct case management and supporting young people to achieve their goals.
SW5 positions provide opportunities to work with more complex cases, mentor practitioners and contribute to practice leadership across the program.
You will bring:
- Qualifications in Social Work, Welfare, Family Therapy or a related discipline.
- Experience in youth work, case management, community services or out-of-home care.
- Strong communication and relationship-building skills.
- A commitment to child safety, inclusion and culturally responsive practice.
