Churches of Christ is seeking an experienced Care Manager to join our residential aged care team at Moonah Park Aged Care in Mitchelton.
This is a key clinical leadership role where you will support high-quality care delivery, strengthen clinical practice and lead teams to deliver safe, person-centred care.
Located in Brisbane’s inner north, Moonah Park offers an easy commute, with the service within walking distance of both Oxford Park and Mitchelton train stations.
About the Role
As Care Manager, you will provide expert clinical leadership across the service, working closely with the Service Manager, Registered Nurses, care staff, allied health professionals and external providers.
In this role, you will:

Lead and support clinical and care teams;

Coordinate resident care planning and quality activities;

Monitor clinical programs, nursing practice and care delivery;

Review AN-ACC documentation and data for accuracy;

Oversee resident documentation and clinical care requirements;

Support compliance with the Aged Care Quality Standards, clinical governance and reportable incident obligations;

Promote a culture of safety, accountability and continuous improvement.
About You
You are a confident clinical leader who brings warmth, accountability and strong communication skills to your work. You build trust with residents, families and staff, while maintaining strong clinical governance and quality standards.
To be successful, you will bring:

Current AHPRA registration as a Registered Nurse;

Minimum 5 years’ post-graduate experience in aged care or an acute clinical setting;

Proven leadership and management capability;
-
Strong communication, problem-solving and decision-making skills;

Intermediate computer skills;
-
Current CPR certificate & First Aid

National Police Check, or willingness to obtain;

Current Australian Work Rights (Sponsorship unavailable for this role)
Postgraduate qualifications in gerontology, dementia or aged care will be highly regarded.
