Job Duties:
- Provide practical, emotional, and community-based support to individuals and families in accordance with individual support plans, well-being goals, and organisational policies.
- Assess participant and family needs and assist with accessing relevant community services, educational programs, training opportunities, healthcare support, housing assistance, and welfare resources.
- Support individuals to develop independent living skills, confidence, resilience, and social participation through structured capacity-building and community engagement activities.
- Assist participants to access available community resources and emergency assistance services, including food relief programs, transport assistance, counselling services, and crisis support referrals.
- Advocate on behalf of participants and vulnerable community members to promote social inclusion, community connectivity, equal access to services, and improved wellbeing outcomes.
- Encourage and support individuals and families to actively participate in social, recreational, educational, cultural, and community activities that promote healthy and independent living.
- Identify immediate well-being concerns, behavioural issues, mental health risks, housing instability, or other support needs, and refer participants to appropriate healthcare, welfare, or crisis intervention services.
- Provide participants with transport assistance and support to attend medical appointments, community programs, educational activities, employment services, and recreational outings.
- Work collaboratively with healthcare professionals, community organisations, and multidisciplinary teams to coordinate holistic support services for participants and families.
- Assist with recovery planning, goal setting, and ongoing support strategies to help individuals achieve greater independence, stability, and community participation.
- Maintain accurate case notes, participant records, progress reports, referrals, and incident documentation in accordance with NDIS standards and organisational procedures.
- Promote safe work practices, participant dignity, cultural respect, duty of care, confidentiality, and inclusive community support practices at all times.
Successful applicants must have the following:
- AQF Associate Degree, Advanced Diploma or Diploma (ANZSCO Skill Level 2).
- At least three years of relevant experience may substitute for the formal qualification.
- Experience in disability support, community services, aged care, or related support environments.
- Understanding of NDIS standards, participant care practices, and community support frameworks.
- Ability to work collaboratively with healthcare and multidisciplinary teams.
- Strong written communication and client documentation skills.
Pay: $79,500.00 – $90,000.00 per year
